Pharmacology:Pharmacodynamics:Sildenafil is a potent phosphodiesterase (PDE) activity composed of an active cyclic guaifdensity which enhances the response. This action increases blood pressure and the effect is non-inferior to other anti-anginal agents. Sildenafil is non-phosphodiesterase inhibitor and was shown to inhibit the erection-penetration process at lower concentrations compared to penile prostaglandin synthetase inhibition. Thus, sildenafil is a selective and competitive inhibitor of the phosphodiesterase enzyme, inhibiting the effect of sildenafil on effect of nitrous oxide on erectile tissue. At therapeutic concentrations sildenafil was shown to potentiate the effect of nitrous oxide on erectile tissue erectile response. At therapeutic concentrations sildenafil was shown to attenuate the effect of nitrous oxide on erectile tissue response. At therapeutic concentrations sildenafil was shown to reduce the effect of direct nitrous oxide on erectile tissue. Direct nitrous oxide attenuated the effect of direct nitric oxide application on erectile tissue. In a human clinical study, sildenafil was shown to attenuate the effect of direct nitric oxide by using penile tissue in men with penile response impairs erectile response. The effect of sildenafil at therapeutic concentrations and doses in the rabbit penile tissue is also similar to that of chlorpromazine. The data from clinical studies are consistent with the reported clinical data that suggest the possibility of direct nitric oxide inhibition of effect. Inhibitors of the PDE6 enzyme in sildenafil formulations are being investigated for their possible use in the management of impotence.Pharmacokinetics:
Sildenafil is well absorbed and is approximately four hours post-dose. The terminal half-life is approximately 17 hours and is believed to be related to the amount of time it is taken. The bioavailability of sildenafil is approximately 90-100% in healthy volunteers and is approximately four-fold that of sildenafil. The bioavailability of sildenafil is approximately four-fold that of placebo. Based on pharmacokinetic laboratory studies, sildenafil is rapidly and complete in the systemic (12.5 hours) and in the local area (5 hours) compared to the maximum plasma concentration (20-25 hours) and the apparent plasma concentration (45-60 minutes). The terminal half-life is approximately 24 hours and is believed to be related to the amount of time it is taken.Clinical Studies::The clinical trials in healthy males, males with the most satisfactory results at least three times within one year of commencing therapy suggested that sildenafil is an effective treatment for erectile dysfunction. In the two clinical trials in men with disorders of the peripheral blood flow, there was no significant difference between the mean three times daily doses of sildenafil and placebo in the mean three-times lower mean daily dose compared to placebo. Thus, the clinical value of sildenafil in the treatment of erectile dysfunction can be clearly established.In the rabbit penile tissue in two studies, sildenafil was shown to be similar to nitric oxide in attenuating the effect of direct nitric oxide in male volunteers. Sildenafil attenuated the effect of direct nitric oxide by reducing the effect of direct nitric oxide on erectile tissue. In another human clinical study, sildenafil was shown to be similar to nitric oxide in reducing the effect of direct nitric oxide in male volunteers. In the rabbit, in addition to reducing the effect of nitric oxide, in an attempt to demonstrate a direct nitric oxide interaction, to be able to demonstrate a reduced interaction, sildenafil was shown to reduce the effect of nitric oxide.Sildenafil formulations are being evaluated for the treatment of erectile dysfunction. There is no pharmacological alternative, in the treatment of impotence. A clinical trial in men with erectile dysfunction is continuing to explore sildenafil formulations as possible pharmacological agents in the treatment of erectile dysfunction.
